Use of Information: Your phone number can be used to contact you via SMS regarding appointment scheduling, inquiries, or customer care related to your services. Data Sharing: Your phone number and any opt-in data collected will never be sold, shared, or disclosed to third parties or affiliates for marketing purposes. Your information is kept confidential and secure. Opt Out: You can opt out of receiving SMS or emails at any time by replying “STOP.” Data Security: Your personal information is protected with appropriate security measures and is only retained for as long as necessary to provide you with our services. This form is not for crisis services.
Please note that Integreative Therapy is a private pay practice and is not in-network with any insurance providers. However, I am happy to provide you with a receipt for services (often called a "superbill") that you can submit to your insurance company for possible reimbursement, if you have out-of-network benefits. Please contact your insurance provider directly to learn more about your specific out-of-network coverage and reimbursement options.
